Influenza A Forms for Brazil Travelers
I flew into São Paulo yesterday and, on the plane, had to fill out the new Influenza A (H1N1) form required by the Brazilian Ministry of Health from all entering Brazil.
The form has symptom check boxes and requests information about destinations and places where you've been recently. A tear-off cover has Influenza A recommendations in Portuguese, English and Spanish. The form must be handed in to a Ministry of Health official stationed before the Federal Police (immigration) booth.
